چکیده

T he more we learn about spirometry, the more we could be challenged. Au contraire, farmers are proficient about horses and are experienced in identifying young thoroughbreds with potential talent. They refer to the horseracing effect by saying that, if you train a fine, young stallion, you might well end up with a winner; and that horses who are leading a race, at a point closer to the finish line, are more likely to win it than those leading at earlier parts of the track. Mathematically, this effect is referred to as co-linearity; your baseline position determines your likely future position. In respiratory medicine, we know about the horse-racing effect from the famous pulmonary curves of FLETCHER and PETO [1], a model postulated 40 yrs ago but not yet validated. It could be assumed that by serially testing with spirometry those at a high risk of chronic obstructive pulmonary disease (COPD), such as heavy smokers, you could identify an increased slope of forced expiratory volume in one second (FEV1) decline. This increased decline could be used as a screening tool for those who finally develop the disease. In 1981, this was named as the lung function horse-racing effect [2]. As stated by FLETCHER and PETO [1], ‘‘In a race between fast and slow horses ... one would expect to find the faster horses out in front halfway through the race’’.

Isoelectric Focusing and PCR-RFLP Joined Techniques for Alpha1-antitrypsin Deficiency Detection

53 persons suspected to alpha1-antitrypsin deficiency detection (AATD) were investigated for ZZ, MZ, ZS, SS, and MS alleles analysis by serum protein electrophoresis (SPE), measurement of trypsin inhibiting capacity (TIC), isoelectric focusing (IEF), polymerase chain reaction (PCR), and IEF/PCR-RFLP techniques. The result clearly shows by using SPE and TIC techniques only 35.85 % and 50.08% of ...

Prevalence of Alpha-1 Antitrypsin (A1AT) Deficiency among Patients with COPD in Kerman, Iran

Background: One of the genetic risk factors for chronic obstructive pulmonary disease (COPD) is deficiency of Alpha-1 Antitrypsin (A1AT). There is no exact statistics about the prevalence of this disease in different regions of Iran. The present study aimed to determine the prevalence of alpha-1 antitrypsin (A1AT) deficiency in COPD patients in Kerman, Iran. Metho...

Polymerized alpha-antitrypsin is present on lung vascular endothelium. New insights into the biological significance of alpha-antitrypsin polymerization.

AIMS The damage to lung tissue in chronic obstructive pulmonary disease (COPD) may involve the progressive loss of pulmonary vascular endothelial cells. Endothelial binding of alpha1-antitrypsin (alpha1-AT) derived from plasma has been identified, and alpha1-AT deficiency is a known genetic risk factor associated with alpha1-AT polymerization and COPD development. Therefore, in the present stud...

Assessment of Alpha-1 Antitrypsin Deficiency in Patients with Severe Chronic Obstructive Pulmonary Disease

Background and Aims: Chronic obstructive pulmonary disease (COPD) is a kind of pulmonary diseases characterized by chronic obstruction of lung that is in the form of a diffuse narrowing of airways resulting in air flow resistance. Alpha-1 antitrypsin (AAT) deficiency is genetically relatively common risk factor in patients with COPD throughout the world and the exact cause of its prevalence is ...

[Alpha1-antitrypsin deficiency: situation in Spain and development of a screening program].

Studies undertaken in Spain indicate that 9% of the general population aged between 40 and 70 years is affected by chronic obstructive pulmonary disease (COPD). Although tobacco smoke is the causative factor in more than 90% of cases, it is estimated that only 10% to 20% of smokers develop COPD. This may be explained by the existence of genetic or environmental factors that modulate the toxic e...
